Did you check the gasket between the TB and manifold? Rare but they can get cracked, especially if you had the TB off.

i think the leak is too big to be a bad seal between the TB and the manifold if it is allowing the car to rev so high

Also check the line off the back of the manifold on the passenger side that runs to the HVAC.
